Student Health Insurance Plan Enrollments
Please follow the instructions below to submit your insurance enrollment form to the ISSS Insurance Advisor. Enrolled students are automatically billed for health insurance via their student tuition bill and do not need to submit a health insurance enrollment form.
- Select your enrollment form below.
- Complete your form digitally, e-sign, and save all pages as one pdf. Name your file as: LastName_FirstName_EID.
- Upload your enrollment form to the Enrollment Document Uploader.
For current students wishing to add their spouse and/or children to their Student Health Insurance Plan. This coverage must be renewed each semester prior to the deadline listed on the enrollment form.
For current students wishing to add dental coverage to their Student Health Insurance Plan. This coverage must be renewed each semester prior to the deadline listed on the enrollment form.
Insurance Extension after Graduation
For recent graduates wishing to extend their student health insurance plan. You must have been covered under the student health insurance plan for at least the last 6 months to be eligible for this extension. The maximum extension permitted is 6 month.
Note that you only receive one opportunity to extend your insurance enrollment. For example, if you choose to extend your insurance for 4 months, you cannot extend it again for 2 more months. Coverage is non-refundable.
J-1 Faculty and Scholars
Visiting faculty and scholars are eligible to purchase the Student Health Insurance Plan during their stay in the U.S. The minimum purchase available is one month of coverage.
You may choose any start date, and the end date will be the previous day on your last month of coverage. For example, if you wish to purchase 3 months of coverage effective January 5, your end date will be April 4. When choosing your coverage dates, please remember that J-1 faculty and scholars are required to have coverage until the end of the current semester or the end of their program (whichever is sooner).
